You Are Well Waiver, Release, and Agreement

I understand I am entering into this agreement with Rebekah Fedrowitz Enterprises, LLC/You Are Well (here-on-in referred to as “You Are Well”). This agreement applies to any and all courses and programs, and all subsequent communications with You Are Well.

I fully understand that You Are Well does not provide medical services and I am not here for medical diagnostic or treatment procedures. All services and communications provided by You Are Well are restricted to nutrition and health education as it relates to diet, dietary supplements, and behaviors associated with eating. The intent of these services is to provided recommendation that support the physiological and biochemical processes of the human body. This does not involve diagnosing, prognosticating, treating or prescribing remedies for the treatment of disease, or for any act for which a medical license or medical authorization is required.

While nutritional and supplement support can be a compliment to my medical care, I understand nutrition counseling is not a substitute for care by a qualified medical provider. I understand that it is my responsibility to consult with a qualified medical professional before making changes to my diet, prescription medication, lifestyle, and/or physical activities.

I understand that no promises or guarantees have been made regarding the results of evaluation or recommend programs.

I agree to hold You Are Well harmless for claims or damages in connection with our work together.

I am participating in this and any subsequent courses and programs solely on my behalf and not as an agent for any government agency on a mission of entrapment.
